Volunteer Report Form- Instructions

In order to access the volunteer report form, you will have to log in as a volunteer.

How to Log in

1.  On the top left-hand side of this page, you will see the "user functions" box. 
This is what it looks like:



2. In the username box, type "volunteer." In the password box, type "lcaf08." The letters in the password will show up as dots. This is what it will look like after you've typed in the username and password:




Click "login" to enter your information. 

Once you've logged in, click here to go to the form!
